Different Types of Blepharoplasty

Blepharoplasty surgery can be divided into two categories: upper and lower.

Loss of fat and volume support of the brow can result in droopiness and excess skin on the upper eyelid which can descend creating a deflated, tired, and aged appearance. Equally, loss of facial fat and the effects of gravity can develop eye bags and excess skin in the lower lid.

Lower Blepharoplasty

Lower blepharoplasty, also known as lower lid surgery, involves removing excess fat deposits, muscle, and skin through an incision made under the lower lashes. This excess tissue and fat deposits are either removed or repositioned to remove wrinkles, dark circles, puffiness, or bags. 

It is also possible that your plastic surgeon will use a technique in which no external incision is necessary. Trans-conjunctival lower blepharoplasty is made through the inner surface of the lower eyelid, leaving no visible scar.

Upper Blepharoplasty

What to Expect Post Blepharoplasty Surgery

Blepharoplasty is a procedure that boasts an impressively brief recovery period for patients. The standard swelling and bruising discomfort around the eyes usually lasts for 1 week following surgery. Initial discomfort is managed with oral medication, applying a cold compress to the affected area and sleeping with the head elevated.

Throughout the two weeks that follow your surgery, you will attend a couple of post-op care appointments with our RN team. There, your incision site will be properly reviewed.

Blepharoplasty Recovery

Days 1-2:  You should have a friend or family member stay with you. Use cold saline compresses to reduce swelling and bruising. Take pain medication as prescribed by your surgeon, when necessary.

Day 5: Regular activities can be resumed following the rest period.

Week 1: Sleep with your head elevated to minimize swelling. 

Week 2: At this point, patients will have returned to work. Contact lenses can also worn and high-intensity exercise can be resumed.

Month 1Up until this point, UV light should be avoided as much as possible to aid recovery.

Is Getting a Blepharoplasty Painful?

Blepharoplasty is a procedure that is only skin-deep and as a result, there is very little pain incurred during the recovery period. It is normal to feel some discomfort and irritation around the eyelids. However,  this can be easily controlled with oral medication and eye ointments prescribed by your surgeon.

How Do I Know If I Need an Eyelid Lift or Brow Lift?

Because the eyebrow position can affect the appearance of eyelids, it can be difficult for patients to understand whether they need a brow lift or blepharoplasty.

The best way to determine what is right for you is to consider your primary objective. While a brow lift can smooth wrinkles of the forehead and lifts the eyebrows, sometimes correcting heavy eyelids, an eyelid lift solely focuses on the eyelids, whether it be the upper lid, lower lid, or both.

Is There an Age Limit for Blepharoplasty?

With Blepharoplasty, there is no set age. That being said, the majority of patients are 30 and older as it isn’t until one reaches this age that the signs of aging begin to show around the eyes. And because the procedure is often performed under local anesthetic, older patients who may be poor candidates for general anesthesia are permitted to undergo the procedure.

Can I Apply Makeup After Blepharoplasty?

We understand that many patients will want to apply makeup as soon as possible. To minimize the chance of infection post-surgery, it is advised to wait at least 10 days before applying makeup around the eye region. Makeup should be applied carefully with clean brushes and without the use of fingers.

Can I Have Blepharoplasty After Laser Eye Surgery?

Blepharoplasty is a procedure that is possible after laser eye surgery. Although blepharoplasty can be done before or after laser eye surgery, it is preferable that it is done at least 6 months after. This allows for any corneal stress and swelling to dissipate around the eye area.

How Many Times Can You Have a Blepharoplasty?

The results of upper eyelid surgery typically last for at least 5-7 years before showing the effects of natural aging, while lower eyelid surgery rarely needs to be repeated at all. While there is no definitive upper limit to how many times you can repeat upper eyelid lift surgery, correctly identifying the causes of repeat eyelid sagging makes it much easier to choose the most effective treatment for your needs. For instance, a forehead lift might be preferable to another eyelid lift. Only your surgeon can determine the best course of action during a consultation.

Can Blepharoplasty Change the Shape of Your Eyes?

While a blepharoplasty certainly can change the shape of the eye if desired, such a procedure must be conducted with great care and precision. Usually the goal is not to change the shape of the eye, but rather, to rejuvenate the area. The main goal of upper eyelid surgery is to remove any excess skin along with the small amount of fat contributing to the drooping, ‘hooded’ look. Similarly, the goal of lower eyelid surgery is to carefully remove a portion from the fatty compartments contributing to puffiness.

Does Blepharoplasty Make You Look Younger?

If you feel you look older than you actually are, saggy or drooping eyelids can be a major contributing factor. While eyelid surgery will not address other cosmetic issues such as adjacent facial wrinkles, crow’s feet, or dark circles, it can certainly make you appear less tired, more alert, and more youthful–as though you can finally open your eyes completely. During the surgery, tissue and sagging skin will be removed as required, based on the underlying structure of your facial muscles and bones as well as the symmetry of your eyebrows.

What Is a Pinch Blepharoplasty?

‘Pinch’ eyelid surgery is an option for patients with excess skin in the lower eyelids causing sagging along with the appearance of folds of skin resembling wrinkles. During a pinch blepharoplasty, a small incision is made just below the lower eyelashes, near the outer corner of the eye so that any remaining scarring is easily hidden in the natural crease that forms there. Then, the lower eyelid skin is ‘pinched’ as we remove any redundant skin causing wrinkles or sagging. The technique is minimally invasive and eliminates extra skin from the lower eyelids without altering the underlying fatty tissues or muscles.

Are You Put to Sleep for Eyelid Surgery?

An eyelid lift can be performed safely with the use of local anesthesia, conscious sedation, or general anesthesia. Local anesthesia is injected, causing a temporary loss of feeling in the eyelids for the duration of the procedure. Conscious sedation involves the use of a sedative administered either orally or through an IV. Oral sedatives relax the patient, who remains awake, while IV sedation causes the patient to doze on and off while remaining responsive to verbal prompts. With general anesthesia, the patient is completely asleep and unresponsive. Regardless of the type of anesthetic decided upon at your consultation, local anesthesia is used in combination to ensure maximum comfort throughout the procedure.

Can Droopy Eyelids Affect Your Vision?

Not only can droopy eyelids make you appear sad or tired, but they can also interfere with your eyesight if they droop too low. The two conditions which cause this are dermatochalasis and ptosis. Dermatochalasis is typically linked to aging and loss of skin elasticity, and presents as excessive eyelid skin. An advanced case can limit your field of vision, as well as cause puffiness or swelling. Ptosis on the other hand is usually caused by muscle weakness. When the levator muscle in the eyelid is not working properly, the lid begins to droop, impairing vision. It can occur at birth or as the result of aging. If you find yourself repeatedly raising your eyebrows in an effort to lift your eyelids, you may have ptosis.

Can I Watch TV After Eyelid Surgery?

It’s important to rest your eyes in the days following eyelid surgery. Post-op, your eyes will get tired easily and your eyelid may feel sore and tight. Your eye might tear up easily, be overly dry, sticky, itchy, or light sensitive. Rather than reading, using your Smartphone or tablet, doing computer work, or watching TV during the first days of your recovery, opt for activities that don’t use the eyes so heavily. The majority of patients are able to watch TV again after a few days. Remember to get enough sleep as being well rested will help you recover more quickly.

How Do You Sleep After a Blepharoplasty?

It is very important that you keep your head elevated for at least two weeks following your blepharoplasty surgery. You can accomplish this easily by sleeping in a recliner at 45 degrees, or sleeping in bed with a foam wedge or 2-3 pillows beneath your head. Always avoid rolling onto the side of your face by sleeping on your back with the help of an airline pillow or the popular ‘husband’ pillow which boasts supports under the arms for added comfort.

What Is a Transconjunctival Blepharoplasty?

One of the most common age-related complaints is puffiness or bags under the eyes. A transconjunctival blepharoplasty is a technique specifically geared at improving the appearance of the lower eyelids. The technique, which has a low complication rate, has gained popularity because it makes an incision inside the lower eyelid, effectively concealing any scars and resulting in a more natural look. Any excess fat below the eye which is contributing to a puffy appearance is then removed through this hidden incision. Due to the location and nature of the incisions, they usually heal within a day and there is no need for stitches.

How Is a Lower Blepharoplasty Performed?

If you are undergoing blepharoplasty surgery on both your upper and lower eyelids, your surgeon will typically work on your upper lids first. Once it’s time to operate on the lower lids, your surgeon will make an incision just below the lashes, within the natural crease of your eye, or inside the lower lid.
